Can you use a TENS unit after knee replacement?
TENS is a safe and effective way to reduce pain after a total knee replacement. Transcutaneous Electrical Nerve Stimulation also known as TENS offers a way to reduce pain without medication. TENS may be used during knee range of motion exercises as well as while icing a painful knee.
Is TENS Safe To Use After Knee Replacement Surgery?
According to the Journal of Sports & Orthopedic Physical Therapy “the group treated with TENS at sensory threshold had a mean decrease in pain of approximately 50% on the VAS compared to a 38% decrease for patients using TENS at subthreshold intensity.”
Finding ways to reduce pain while improving function may produce huge benefits to a patient’s post-surgical outcome.

How long after a knee replacement can I start using my TENS unit?
As long as you have been medically approved by your doctor to use a TENS unit it is safe to start using one the same day as surgery.
It is important to use a new set of electrodes to reduce the chances of infection.
The electrodes may be placed on either side of the knee incision.
According to Rehab Management – “Pain management is a significant factor in postreplacement rehabilitation.”
TENS is a safe and effective way to reduce post surgical pain without increased medication.
If you are using cold therapy the TENS electrodes may be placed directly on the skin under the ice pack or above and below the ice pack.
If you are experiencing thigh pain due to the tourniquet then the TENS unit may be used on the mid thigh with or without an ice pack.

Is TENS Safe To Use While Exercising?
In this video I demonstrate how a TENS unit may be used while using a knee flexion/extension machine.
This machine uses hydraulic resistance.
The client in this video underwent a total knee replacement 6 weeks before this video and underwent a manipulation under anesthesia 3 weeks after surgery.
Is TENS Safe To Use With A CPM (Continuous Passive Motion Machine)?
Yes, using a TENS unit during use of a CPM is a great way to improve the effectiveness of this device. Adding an ice pack with TENS is a popular combination.
Set the TENS unit to turn off after 15 minutes of use. This reduces the chances of experiencing skin irritation and allows the central nervous system to remain sensitive to the sensation.
A commonly prescribed strategy is to use the TENS unit for 15-minutes followed by a 15-minute episode with the TENS unit in the off position. This cycle may be repeated as long as one is on the CPM.
How To Use TENS During Range of Motion Exercises?
During knee flexion the TENS unit may reduce pain and reflexive muscle guarding. In the photo below I am demonstrating a technique known as a heel slide with overpressure.
To perform this exercise the client would sit on a bed, couch, or floor with the foot placed flat on the surface. After sliding the foot back toward the hip, hands will be placed at the top of the shin bone and gentle pressure will be applied back toward the body.
This position and pressure will be sustained for 10-seconds. Next, the pressure will be removed for 10-seconds then the cycle will repeat.
